During the first visit, we make sure to obtain important background information, like your medical history, and give you time to get to know your doctor. Mission Statement
It is the mission of our practice to pursue the health and well-being of our patients by providing the highest quality medical, surgical, and DMEPOS care in a safe, effective, and efficient manner. We provide excellent patient service and value the needs, wants, and perspectives of our patients.
To this end, it is our goal to operate this practice in accordance with all applicable laws and regulations and to ensure availability of necessary facilities, supplies, products, and start with the technical expertise to meet the physical and mental needs of our clients.
Our mission and goals are also documented in the employee manual to ensure that all employees, including new employees, can have a copy for regular reference. From that mission statement, senior management has developed a constellation of goals and objectives for the practice to help us meet our mission. This mission statement is periodically reviewed to ensure that it reflects the current true mission and that day-to-day actions are commensurate with our organizational goals.

What to expect
Being well-prepared for your appointment will ensure that the doctor has all of the needed information to provide the best possible care for you. It also will help relieve any unnecessary anxiety you may be feeling. Please bring your insurance cards and photo ID with you to your first appointment, and if you haven't filled out the forms yet, please arrive at least 15 minutes early to fill out the patient information.
Appointments
It is customary for our practice to make appointments in advance. We prefer to schedule all appointments through our centralized scheduling office. To make an appointment please call 1-800-FOOT-DOC or 606-679-2773. If you find the need to reschedule, please call us at your earliest opportunity so that we may offer that appointment to another patient.
Insurance
Our office accepts all major insurance plans, as well as most of the smaller carriers. As a courtesy, our office will file insurance claims on your behalf. It is important to note any copayments, deductibles, or fees for service not covered by insurances will need to paid at the time of your office visit.
